After centuries of banishment following her conflict with Inarius, the Succubus Queen re-emerges to reclaim her creations from the influence of both the High Heavens and the Burning Hells. This event serves as the catalyst for the Fourth Prime Evil campaign, forcing the Nephalem to choose between salvation through order or strength through chaos.

Lore & Background
Lilith was forged in the Burning Hells by Mephisto before fleeing with Inarius to create Sanctuary using the Worldstone. While Inarius sought to protect humanity from demonic influence, Lilith believed that giving humans their own power was the only way for them to survive the eternal war between Heaven and Hell. She was eventually banished to the Void by her former partner after refusing to abandon her children to angelic control. Her return is precipitated by a ritual orchestrated through the research of the ancient necromancer Zoltun Kulle, who sought to harness the essence of the Prime Evils. The Whispers, manifestations of Mephisto's influence, guide the player to gather these essences, inadvertently facilitating Lilith's resurrection. She views her return not as an invasion, but as a rescue mission intended to liberate humanity from the tyranny of angels and demons alike.

The air in the ruined temple tasted of ash and old blood. A cultist knelt before the obsidian altar, his hands trembling as he poured the vial into the basin. The liquid did not splash; it hissed, rising like smoke to form a silhouette against the dying light. 'She is here,' the cultist whispered, knowing he would never see dawn again. From the shadows of the altar, eyes ignited with crimson fire, burning away the centuries of exile.
Reader's Guide
The narrative begins as the Whispers draw the Nephalem across Sanctuary to locate the essence of the Prime Evils. As the player progresses through regions like Fractured Peaks and Kehjistan, they uncover Zoltun Kulle's journals detailing a ritual intended to summon Lilith. Upon collecting the necessary components, the ritual completes at the Cathedral of Light in Scosglen. Lilith manifests physically, revealing her intent to rule Sanctuary by empowering humans with demonic strength. The Nephalem must confront her directly in a final battle atop the cathedral ruins. The conflict forces a choice regarding humanity's future: allow Lilith to lead them toward war and power, or defeat her to prevent the corruption of their souls. Ultimately, the player strikes down the Mother, ending her immediate threat while leaving Sanctuary vulnerable to other evils.
